Position Details
Position: Registered Nurse Case Manager (Home Health)
Location: Glendale, AZ Area
Schedule: Monday-Friday | 8:00 AM-5:00 PM
Paid Clinical Orientation: 2 Weeks
Mileage Reimbursement Included
Competitive Pay-Per-Visit Rates
Routine Visit: $60
Start of Care (SOC): $112
Recertification: $80
Discharge Visit: $70
Evaluation Visit: $55
Non-Visit Activity (NVA): $40
What You'll Be Doing
Conduct comprehensive patient assessments in the home setting
Develop and manage individualized plans of care
Administer medications and treatments as ordered
Educate patients and families on disease management and care plans
Coordinate care with physicians and interdisciplinary teams
Monitor patient progress and adjust care plans as needed
Support patients through recovery, rehabilitation, and improved quality of life
Qualifications
Active RN License
Minimum 1 year of Acute Care Nursing Experience
Current BLS Certification (Hands-On CPR)
Valid Driver's License & Auto Insurance
Home Health Experience Preferred but Not Required
Associate's or Bachelor's Degree in Nursing

Pay Details: $60.00 to $80.00 per hour
